The steps
File the Medicaid application
Apply online at Georgia Gateway or with the Centralized TEFRA/Katie Beckett Medicaid team. Parents are listed as authorized representatives.
Get the medical packet
You'll receive forms DMA-6A, DMA-706 and DMA-704. You and your child's doctor complete them.
Gather supporting records
When needed, add a psychological or developmental evaluation no more than three years old, plus therapy notes, an IFSP or an IEP.
Mail everything together
Send the full packet to Alliant Health Solutions (address below). Keep copies of every page.
Level of care, then eligibility
If Alliant approves the level of care, DFCS finishes the Medicaid decision. Approval can include retroactive months.
The forms
| Form | Name | Who fills it out |
|---|---|---|
| DMA-6A | Physician's Recommendation | Your child's doctor |
| DMA-706 | Medical Necessity / Level of Care Statement | Doctor and caregiver |
| DMA-704 | Cost-Effectiveness Form | Doctor and caregiver |
Signatures must be original, not stamped. The doctor's signature is only good for 90 days, so send the packet promptly.

Tips that save time
- Ask the doctor's office to describe your child's daily care in detail. "Needs suctioning every 2 hours overnight" beats "needs suctioning."
- Send the whole packet at once. Missing pages are the most common delay.
- Write your child's name and date of birth on every page.
- Open every letter from DFCS and Alliant right away. Some have short deadlines.
